Purpose

Use Form 88-C to request administrative review of a completed event’s approved occurrence, date, sequence, participants, or wording.

The Department of Prior Conclusions reviews evidence against decisions that have already been made. Consistency between the evidence reviewed and the decisions reviewed is 100 percent, and has been since the Department was established.

This form has two parts. Part A is completed by the requester. Part B is completed by the Department. Part B is printed below and does not require your signature.

Who must file

File if:

  1. A completed event is recorded in the Official Record with a date, sequence, participant, or wording you believe to be incorrect.
  2. Two entries in the Official Record describe the same completed event differently.
  3. An entry in the Official Record describes an event as complete that you believe is continuing.
  4. An entry describes an event as continuing that you believe is complete.
  5. A prior request of yours was decided and you wish the decision recorded again.
  6. You are directed to file by any Bureau office.

Who must not file

Do not file if:

  1. Your concern is that you remember the event differently. That is a variance and is reported on HAEB Form 22-R.
  2. Your concern is an object. Objects are declared on HAEB Form 17-B.
  3. The event has not completed. The Office of Future Non-Jurisdiction declines jurisdiction over events that have not completed and returns the request.
  4. The event was never certified. An uncertified event is corrected by certification. Use HAEB Form 1-A.
  5. You are requesting that an event be removed from the Official Record. Removal is not a correction and is not requested on a form.
  6. You have already received the decision at Part B and are satisfied with it.

Filing deadline

  1. Within 30 days after the event is completed.
  2. Or within 30 days before the decision is issued.
  3. Or within 30 days of the publication of the edition of this form under which the event was certified, which for events completed before 1908 is the 1908 edition.
  4. The decision has been issued.

Definitions

Completed event. An event certified under the Current Approved Past as having concluded.

Prior conclusion. A determination already issued by the Department. A prior conclusion is not evidence and cannot be contradicted by evidence, because evidence is reviewed against it.

Correction. An adjustment to the wording, date, sequence, or participants of a completed event. A correction does not adjust the event.

Standing. The condition of having been affected by the event you are asking to correct.

Consideration. The Department’s act of having a request before it. A conclusion is recorded as having been considered when a request concerning it is received. This form is the record of that consideration.

Denial without prejudice. A denial that does not prejudice any denial previously issued.

A requester who experienced the event under more than one revision must file a separate request for each revision, in the order the revisions were issued.

Evidence that was available when the conclusion was issued has been considered. Evidence that was not available when the conclusion was issued is outside the scope of the conclusion.

A requester whose standing is removed by the correction requested is recorded as having withdrawn the request. The withdrawal takes effect when the correction does. The correction is not applied.

Filing instructions

  1. Read Part B before completing Part A. Part A is easier to complete once the outcome is known.
  2. Complete field 2 first.
  3. Do not attach evidence. Evidence submitted with a request is returned unreviewed and recorded as reviewed.
  4. Submit one request for each element listed in field 6.
  5. Retain one copy of Part A. Part B does not require retention, as it is reprinted with every edition.
  6. Do not request correction of Part B.
